Multidisciplinary Approach
Unlike some other local outpatient clinics, CHOICE is a multidisciplinary clinic that can include physical therapy, occupational therapy, and speech therapy, depending upon our patients’ needs.
People who receive outpatient rehabilitation at CHOICE Therapy Services have access to rehabilitation professionals who communicate and work together for the most positive treatment outcome possible. Rehab staff that may join the patient’s team as necessary include:

Collectively we have more than 40 years of experience working with all ages and all types of conditions in the physical rehabilitation services. Our therapists have personal experience with overcoming barriers of disease and physical challenges themselves, both as patients and as caregivers.
Individualized Care and Goal-based Outcomes
At CHOICE Therapy Services, we focus on individual goal-based outcomes, working with our patients to determine their short- and long-term therapy goals to help them transition back to work, school, and/or other activities of daily living.
Each therapy program begins with a comprehensive evaluation and development of a treatment plan based on a physician’s order and the patient’s particular goals, circumstances, and lifestyle.
Treatment programs are designed to meet each patient’s specific needs. Rehabilitation can address a specific need with a single therapy, or it may require a multidisciplinary approach to address a broad range of needs. For example, a stroke survivor may need to strengthen a weakened side and enhance memory and thinking skills.
Coordinated Care
We offer overall care planning and coordination as part of our services. Referrals may be made to additional providers such as:

- Case Manager
- Dietitian
- Neuropsychologist
- Rehabilitation Physician
- Other Specialists as Needed
To ensure continuity of care across offices and facilities, regular communication occurs between all team members, including YOU, your family, your referring physician, and any other ancillary or specialty providers you may see.
